MVInet Account Information and Forms

Email all completed forms to your TxDMV Designated Contract Representative (DCR). If you do not
know or have a DCR, email the forms to VTRDataContracts@txdmv.gov.
 
MVInet Account Update Request form - Please use this form to update general business
information, add or delete MVInet Security Administrators, or edit your billing contacts.
 
MVInet Authorized User form (VTR-MVIU) - This form is required to add, delete, or change an
authorized user's name (following a legal name change). An MVInet Security Administrator
must complete, sign and date this form.
 
Data Use Agreement (DUA) - All authorized users are required to read, sign and date this
form (new and name change only). The MVInet Security Administrator must ensure this form is
properly executed by the user and must be included when submitting a VTR-MVIU. A DUA is not
required when deleting a user.
